7th and 8th Grade Sports
General Information
Physicals. Students wishing to participate in a sport must submit a "Athletic Physical Form" signed by a medical doctor before the sports season begins; forms are available in the Main Office . Completed forms, with all requested signatures, must be returned to the Blevins Health Office before the student will be permitted to practice. All physicals expire one year from the examination date. Physicals are not needed for tennis or golf.
Fees. Seventh and eighth grade students must pay a $60 per sport participation fee ($250 per year family maximum). This fee is waived for students who meet federal guidelines for the free or reduced lunch program after a completed application form is returned to the school office.
